General Information
Payment
Fax or mail a completed registration form which may be found on the website or on the back of any ULSD Course Brochure (Master Card, Visa, or a check may also be used) made payable to the University of Louisville, and mailed to : Continuing Education, Room 123P, School of Dentistry, University of Louisville, Louisville, KY 40202-1701. A registration form may also be obtained from the ULSD Continuing Education office.
Hotel Accommodations
Participants are responsible for making their own hotel reservations but the C.E. office is happy to provide a list of hotels in close proximity to the school. Inform the hotel that you are with the U of L group when making a reservation. You may qualify for a reduced corporate rate.
Course Credit
The courses offered by the University of Louisville for dentists and dental hygienists are accepted by the Kentucky Board of Dentistry and virtually all other licensure boards for credit of one point per hour to meet re-licensure requirements. ULSD is an ADA CERP Recognized Provider. All these courses are approved for Fellowship and Mastership credit with the Academy of General Dentistry.
Academic Freedom and Professional Differences
Educational institutions have the responsibility to disseminate new knowledge. In so doing, some presentations may include controversial procedures, materials, or commercial references. Sponsorship of a dental continuing education course by ULSD does not imply endorsement of a particular philosophy, procedure, or product. Continuing education courses do not necessarily reflect the philosophies of the University of Louisville pre-doctoral or post-doctoral training programs.
Co-Sponsored Courses
ULSD offers some courses co-sponsored with other organizations, including some commercial companies. Such financial assistance allows us to afford nationally prominent speakers and to offer courses at lower fees. ULSD will identify co-sponsorship in program promotional material and will seek assurance by such speakers that balance and unbiased information.
Registration and Cancellation
Participants are requested to pre-register by credit card or check at least TWO WEEKS in advance (four weeks for travel courses). Decisions regarding cancellation of courses will be made at this time.
Participants desiring to register late or at the door are advised to first telephone (502) 852-5077 to check on availability.
CANCELLATION & REFUND POLICY:
The ULSD reserves the right to cancel any course should circumstances warrant such action. In the event a course is cancelled or filled, the ULSD will refund the tuition. When a participant pays a registration fee, if the customer request cancellation two weeks in advance, the fee may be returned. In lieu of reimbursement, or for cancellations received less than two weeks in advance, the registration fee may be applied toward another course.
Course Registrants who don’t attend or give notice of cancellation are responsible for the course tuition.
FOR LIMITED ATTENDANCE COURSES, cancellations received two weeks or less before the course date will result in the loss of tuition or deposit.
Course Changes
The ULSD reserves the right to modify the course content, program faculty and program activities and reserves the right to cancel course offerings where necessary. Occasionally course dates may be changed to accommodate speakers' schedules or other scheduling conflicts. Pre-registered participants will be notified if a course will not be presented.
Course Schedule
Unless otherwise specified, courses begin at 9:00 am. Plan to arrive one-half hour prior to course time to register and pick up information packets.
Reduced Tuition
ULSD, faculty, staff, students, residents and members of the School's Second Century Society may qualify for reduced tuition for certain non participation, non-travel courses. Please contact us for specifics.
