Once a laser accident involving injury has occurred, Radiation Safety will conduct an accident investigation. The investigation will involve review of the laser set up, any safety precautions that were in place and evaluation of the state of training of lab personnel. The focus of the investigation will be for “lessons learned” and the correction of any issues that contributed to the accident.
