Important Notice to All Principal Investigators

All work involving biohazardous agents, recombinant DNA molecules, use of human blood, body fluids or tissues, gene therapy, use of USDA agents or CDC select agents must be registered and in some cases approved by the Institutional Biosafety Committee (IBC) prior to initiation of work.  It is the responsibility of the Principal Investigator to submit a completed IBC application/registration form to the Biological Safety (BS) Program Office at biosafe@louisville.edu.

In order for applications/registrations to be considered on the monthly agenda, an initial electronic draft of the application/registration form must be received by the BS Program Office by the deadline listed below. The deadline allows sufficient time for revision of the draft submission to ensure an administratively complete application is available to the committee in an effort to expedite review and approval. Incomplete submissions may necessitate a delay in consideration to a subsequent meeting. If there are no IBC applications/ registrations submitted by the monthly deadline, the standing IBC meeting may be cancelled.
