Beck Depression Inventory II (BDI-II)	Clients complete the form on their first visit and then after at least one month (or more) of counseling.	See SLO#1.	To Be Determined.
Client Feedback Questionaire	Clients who chose to complete the form after their first or second visit.	Positive results; average score of 6.5 on a 7 pt. Likert scale.	Used in annual evaluation of staff Used in the supervision of practicum students
Counseling Center Evaluation Questionaire	Clients who chose to complete the form after their third visit or a later visit.	Positive results; average score of 5.8 on a 7 pt. Likert scale.	Used in annual evaluations of staff Used as a part of the assessment of formal learning objective. Used in the supervision of practicum students Used to assess the effectiveness of services with respect to client behavior changes and academic performance.
