CODRE Application Evaluation Form

A completed Program Evaluation Form is due 30 business days after the conclusion of the program. Failure to provide an evaluation form with final versions of publicity and a list of everyone who attended the CODRE co-sponsored event could result in a loss of funding for a period of one year. Submit your latest publicity, original receipts and any photos or graphics taken at the event to service account.

Any programming event that does not meet the guidelines or support programming committee goals and University diversity plans may not be funded. The Diversity Programming Committee of CODRE reserves the right to deny any request that does not meet the guidelines or support programming committee goals and/or University diversity plans.
