To receive intervention services at STAR, you must have an autism spectrum disorder diagnosis.  If you have not had a diagnostic evaluation, please contact the Weisskopf Child Evaluation Center at (502) 852-5331.

If you do already have a diagnosis, you may begin the referral process.  We do require a doctor's referral, which can be written on their prescription pad.  It must include:

• Patient's Name
• Parent/Legal Guardian's Name(s)
• Parent/Legal Guardian's Contact Phone
• "Refer to STAR"
