Referrals

• The Genetic Services Referral Form must be received from a primary care doctor or specialist before setting up an appointment.
• Once we have received your referral, it will be processed and you will be contacted to set up an appointment.
• It may be several weeks before your appointments can be scheduled.
• After the appointment is scheduled you will be mailed a reminder letter and an application to be completed and brought to your visit.
