Opt-Out

Major Medical Fee Waiver Process

If you do not want the Student Health Insurance Plan, you must decline or opt-out of coverage by submitting a waiver. You may only opt-out of coverage during the following Waiver Periods. If your waiver is approved, it is with an understanding that you will be covered for the entire fall/spring semester. If you lose coverage, you are responsible to enroll in a qualifying event. Domestic Health Professional Students:
Major Medical Fee Waiver Process

All Dental, Medical, Nursing, Dental Hygiene, Audiology and Speech Pathology students are required to be covered by a health insurance plan. The Waiver process for Summer 2024 will start April 17th. Watch for the email from Academic Health Plans in your University of Louisville student email account

Deadline for Summer Waiver is June 5th, 2024.

*Please note: Healthcare Sharing plans and Out of State Medicaid plans will not be accepted to waive the student health insurance plan.

 

International Students

All students with an F1 or J1 Visa are required to be covered by a health insurance plan. The opt out/waiver process for Summer 2024 will start April 17th. Watch for the email from Academic Health Plans in your University of Louisville student email account that will provide you the link to opt out.

For an international student to be eligible for a waiver the following criteria must be met:

    Criteria:

  1. Must be a US based employee plan (can be the subscriber or dependent, or the students home country provides US insurance coverage, or fully financially sponsored by US or other international organization which provides health insurance backed by a U.S. health insurance carrier, and,
  2. Must be ACA Compliant, and
  3. Must provide Certificate of Coverage and copy of insurance ID card, and
  4. Coverage must include medical evacuation and repatriation benefits. Minimum amount of coverage must be 100,000, and 25,000 respectively. If this coverage is missing or amount is not met, the student will be advised to purchase this coverage from AHP’s website
