Revised 2015 Title IX/Clery Syllabi Statement
As a result of feedback from faculty and consultation with legal counsel, please use this REVISED statement for inclusion in all course syllabi, effective July 1, 2015.

Faculty and Staff: Complete Attestation and Disclosure Form
All faculty and staff are required to complete an Attestation and Disclosure form in iRIS on an annual basis according to UofL policy. For instructions, go to A&D Form.
