Open Enrollment Checklist
Before Open Enrollment
- Review your 2007 health, pharmacy, vision and dental expenses and current benefits selections to determine if you want to make changes (see the letter HR mailed to your home for your current benefit elections).
- Review the dependent data provided in the letter you received at home regarding your current benefit elections. To make changes to your dependent information, log on to ULink (ulink.louisville.edu). Select the Faculty/Staff tab. Select Benefits Enrollment under the Human Resources section.
- Determine whether dependents covered in the past are still eligible for coverage.
- Look at the 2008 benefit options to decide what’s best for you.
- Take the online health risk assessment (available Oct. 22 - Nov. 16) if you want to participate in Get Healthy Now (and save $20/month on your health insurance). HealthFitness Corp. will call you to tell you what to do next.

- Change, add or drop benefits for 2008
- Change, add or drop dependents for 2008
December 2007
- Double check your choices when you receive the benefits confirmation letter and e-mail from Human Resources
- Report errors to Human Resources immediately so that corrections can be in place by Jan. 1, 2008. Call 852-6258 for a correction form to complete or use our secure web form to report errors.
- Watch for identification cards in the mail.
January 2008
- Check your first paycheck of the year to make sure that your selected benefits are being deducted accurately.
