Vision
UofL offers two vision benefits. One is an insurance policy through Anthem Blue View Vision. The other is a discount plan through the UofL Department of Ophthalmology and Visual Sciences.
What do I have to do during Open Enrollment?
- To have vision coverage through Anthem Blue View Vision if you had vision coverage in 2007 and cover the same people: Do nothing.
- To have vision coverage through Anthem Blue View Vision if you had vision coverage in 2007 and cover different people: Enroll.
- To have vision coverage through Anthem Blue View Vision in 2008 if you did not have vision coverage in 2007: Enroll.
- If you had vision coverage through UofL in 2007 but do not want it through in 2008: Once in the Benefits Enrollment portion of ULink, check the box to waive / cancel coverage.
Who is eligible for the vision plan?
- Regular status employees who are employed 80% or greater full-time equivalency (FTE)
- Regular status part-time employees and temporary part-time lecturers (40% to 79% FTE)
- UofL retirees and their spouses
- Spouses of covered UofL employees
- Qualifying adult
- Unmarried child(ren) (natural children, step children, foster children, legally adopted children and children placed for adoption) under age 25 who:
- is a tax dependent of the employee, the employee’s spouse or a qualifying adult and
- receives over one-half of his/her support from the employee, the employee’s spouse or a qualifying adult;
- Unmarried child(ren) of the employee or the employee’s spouse of any age when such children are incapable of self-support because of a total and permanent disability; and
- Unmarried child(ren) under age 25 for whom the employee is required to provide health care coverage under a qualified medical child support order (QMCSO), regardless of where the child resides or if the child is dependent upon the employee for support.
- Surviving spouses under age 65 of UofL employees or retirees.

Anthem Blue View Vision's broad national network consists of more than 44,000 providers and provider locations, including independent optometrists and ophthalmologists, as well as the convenience of retail locations to include LensCrafters, Target Optical, JCPenney Optical, Sears Optical and most Pearle Vision locations nationwide. For Anthem Blue View Vision provider locations, members may access the provider directory on the Anthem home page. If you need more information on coverage, call the Anthem Blue View Vision Customer Service number at 1-866-632-8502 (only until Nov. 2, 2007).

ID Cards
You will receive new ID cards for 2008.
Use: Present to providers in the Anthem Blue View Vision network when you receive services. The in-network provider will submit your claim.

UofL Department of Ophthalmology & Visual Sciences Discount Service
This is a discount service that cannot be used with insurance or any other discounts. A University of Louisville ID card is required. There is no monthly cost.

The Department of Ophthalmology & Visual Sciences offers services through Eye Specialists of Louisville and 20/20 Eye Care. Offices are at the Kentucky Lions Eye Center on the Health Sciences Campus, near Belknap Campus at 215 Central Ave., Suite 101 (Central Station at Jim Patterson Field) and at other locations (go to www.2020icare.com for a list and other information).
