Anthem Dental Blue replaces HumanaDental as UofL's dental provider. Anthem Dental Blue is a PPO plan. You can see any dentist, but will save more money and receive more benefits when you go to a dentist in the Anthem Dental Blue PPO Network. Dentists outside the network can bill you for charges above the amount covered by the Anthem Dental Blue plan. As an alternative to dental insurance, the School of Dentistry also provides dental services.

What do I have to do during Open Enrollment?
  • To have dental coverage if you had it in 2007 and to cover the same people: Do nothing.
  • To have dental coverage if you had it in 2007 and to cover different people: Enroll.
  • To have dental coverage in 2008 if you did not have it in 2007: Enroll.
  • If you had dental coverage in 2007 but do not want it in 2008: Once in the Benefits Enrollment portion of ULink, select waive / cancel coverage during open enrollment.

Who is eligible for the dental plan?
  • Regular status employees who are employed 80% or greater full-time equivalency (FTE)
  • Regular status part-time employees and temporary part-time lecturers (40% to 79% FTE)
  • UofL retirees and their spouses
  • Spouses of covered UofL employees
  • Qualifying adult
  • Unmarried child(ren) (natural children, step children, foster children, legally adopted children and children placed for adoption) under age 25 who:
    • is a tax dependent of the employee, the employee’s spouse or qualifying adult and
    • receives over one-half of his/her support from the employee, the employee’s spouse or qualifying adult;
  • Unmarried child(ren) of the employee or the employee’s spouse of any age when such children are incapable of self-support because of a total and permanent disability; and
  • Unmarried child(ren) under age 25 for whom the employee is required to provide health care coverage under a qualified medical child support order (QMCSO), regardless of where the child resides or if the child is dependent upon the employee for support.
  • Surviving spouses under age 65 of UofL employees or retirees.
  • COBRA participants

  • You can go to the School of Dentistry dental clinic for a full range of general and specialty dental services. Dental students, dental hygiene students and dental residents provide the majority of treatment under faculty supervision. The clinic has lower fees and provides discounts on orthodontia and other services. Call 852-5401 to set-up an appointment. ID Cards

You will receive new ID cards for 2008.

Use: Present to your dentist at the time of service. It has all the information your dentist needs to submit your claim.
