	Theresa Berbet Department of Fine Arts Outstanding Performance Classified Staff
	Sharon O'Bryan Department of Anthropology/Geology Outstanding Performance Professional/Administrative
	Linda Baldwin Department of English Exceptional Service to Faculty and Staff
	Margie Folden Arts & Sciences Dean's Office Exceptional Service to Faculty and Staff

Outstanding Performance Nomination Form -- two categories: Classified and P/A Staff.

• This award acknowledges the outstanding performance of staff in the College of Arts & Sciences. Two awards of $500 will be presented annually, one to a Classified Staff member and one to a Professional/Administrative Staff member.
• Previous award recipients are ineligible to receive this award again for a three-year period.
• Completed nomination forms should be sent to the A & S STAFF AWARDS COMMITTEE, c/o Yolanda Demaree, A & S Dean’s Office. The deadline for nominations is February 16, 2007. Recipients will be announced March 29, 2007 at the awards reception.
